San Francisco opened as a small favorite in early markets, but Tennessee has flipped the matchup and climbed to roughly a 5.5-point road favorite as the Titans confirmed that Cam Ward and their starters will play while the 49ers are expected to rest most of their established first team. That movement makes Tennessee difficult to chase on the spread. The total, now around 40.5, offers a more attractive way to bet the game at Levi’s Stadium.

Tennessee has the most obvious preseason quarterback-depth advantage of these two teams. Cam Ward is the regular-season starter and is expected to play. Behind him, Mitchell Trubisky is listed as the No. 2 quarterback, followed by Will Levis and Hendon Hooker. That is an unusually experienced group for an exhibition, with multiple quarterbacks who have started meaningful NFL games. It explains why bettors moved quickly toward the Titans once the playing-time plans became clearer.

San Francisco is taking the opposite approach. Most regular starters are expected to rest, leaving the preseason opener to a roster focused on depth evaluation. The total has moved much less dramatically, from around 39.5 into the 40 to 40.5 range. That creates an interesting contrast. Tennessee’s quarterback depth should prevent a complete offensive collapse, but the Titans still have little reason to show an aggressive regular-season game plan. Ward needs useful reps, not 30 pass attempts. Once he exits, Tennessee can evaluate its deep quarterback room while leaning on the run game and basic concepts.

San Francisco’s reserve-heavy offense is the stronger reason to prefer the Under. The 49ers have offensive-line questions to evaluate and are expected to protect core skill players. Mac Jones can run an NFL offense, but the personnel around him will change. Younger quarterbacks behind him will face a Tennessee defense that can rotate fresh players and still keep experienced passers on the other sideline controlling field position.

The teams already held a joint practice this week, giving both coaching staffs a long look at opposing personnel. That reduces the incentive to use the exhibition as a tactical laboratory. Tennessee saw San Francisco’s fronts and coverages in practice, while the 49ers tested themselves against Ward and the Titans’ new offense. The game can therefore become more about clean operation, tackling and roster evaluation than scheme.

San Francisco won the most recent regular-season meeting 37-24 at Levi’s Stadium last December. Purdy threw three touchdown passes in that game, while Ward was Tennessee’s quarterback. That result is not a useful reason to expect another high-scoring game because the 49ers’ regular-season offensive core drove much of that production and is not expected to play meaningful snaps tonight.

Matchup Breakdown

Ward is the most dangerous player for the Under. He has already completed a full rookie season as Tennessee’s starter and enters Year 2 with more command of NFL structure. If San Francisco fields a reserve-heavy defense from the opening drive, Ward can create points quickly. The Under needs Tennessee to treat his appearance as a controlled tune-up rather than an extended scoring exercise.

The Titans remain functional after Ward leaves. Trubisky has started dozens of NFL games, while Levis has substantial starting experience of his own. That depth is precisely why the spread moved so far. Yet veteran backup quarterbacks can also support an Under by protecting the football and taking available completions rather than forcing high-risk throws. Tennessee can win the field-position battle without needing 30 points.

San Francisco’s offensive line is more concerning. The 49ers are evaluating competition at left guard and other depth spots, and reserve combinations can struggle to maintain clean pockets. Tennessee does not need to blitz heavily to create disruption if communication breaks down. Sacks, penalties and negative runs are the type of plays that turn a promising preseason drive into a punt.

Weather should not interfere. Santa Clara is expected to be clear and mild with only light wind, and Levi’s Stadium uses natural grass. That means the Under cannot rely on external conditions. The handicap is almost entirely about how quickly the 49ers reach their reserve offense and whether the market has overreacted to Tennessee’s superior quarterback depth by lifting scoring expectations along with the spread.

Game Thesis: Tennessee deserves to be favored because Ward will play and the Titans can follow him with Trubisky, Levis and Hooker, while San Francisco is expected to protect most of its starters. The problem is that the spread has moved so far that bettors are now paying full price for that advantage. The 49ers’ reserve offense is more likely to struggle than explode, and Tennessee can control the game without playing at a high tempo. That makes the Under more appealing than laying an inflated road number.

At 40.5, the game can contain an efficient Ward touchdown drive and still finish Under if the 49ers’ reserve offense struggles.

The primary risk is Tennessee’s quarterback room simply being too strong for San Francisco’s backups. Ward could score early, Trubisky could maintain the pace, and a turnover by a young 49ers quarterback could create short-field points. Even with that risk, the current total asks a reserve-heavy San Francisco offense to contribute enough scoring to push the game into the 40s. That is a larger assumption than the personnel news justifies.
